Cases of fatal thrombosis after coronavirus vaccination caused a stir in March. Vaccines from Astrazeneca and later Johnson & Johnson were suspected of causing so-called sinus and cerebral vein thrombosis. Germany, for example, therefore briefly suspended the Astrazeneca vaccination campaign in the spring.

The Robert Koch Institute (RKI) then recommended that the vaccine be used only in people aged 60 years and older, as this side effect was predominant in younger people. Doctors from Berlin and Greifswald have now found out in a study on how to prevent fatal thrombosis after coronavirus vaccination.

Frequency very rare

“The frequency of this vaccination complication was very rare, but some people lost their lives, which certainly contributed to the uncertainty and general skepticism about vaccination.” stated in the statement for the new study. Symptoms usually begin within about three weeks of the first vaccination.

In this study, the researchers report a series of cases in which people with disabilities complained of a very severe headache a few days after vaccination. Laboratory tests have shown that they have suffered from vaccination complications that can lead to thrombosis – blood clots that clog blood vessels – and an increased tendency to bleed. This is called vaccine-induced immune thrombotic thrombocytopenia (VITT).

Severe headache as a warning

For most people affected, timely and consistent action can prevent serious consequences. Researchers are talking about “pre-VITT syndrome” – a therapeutic period of time that should be used to effectively protect against the dreaded consequences of vaccination, such as cerebral vein thrombosis.

Specifically, the study reported cases of eleven patients who came to the doctor five to 18 days after vaccination with severe headaches. However, it turned out that this was not an accompanying symptom of VITT, but rather an “omen” and a “warning”. This opens up “room for maneuver for early therapeutic interventions,” writes study author Farid Salih of the neurology department of Berlin’s Charité Hospital.

Seven patients did not experience any thrombotic complications. With one exception, according to the publication, all underwent therapy with high-dose immunoglobulins, glucocorticoids and anticoagulants within five days of the onset of headache. In contrast, in all four patients who had already developed thrombosis after the initial presentation, anticoagulation began either late – until day 6 to 9 after the onset of headache – or treatment was stopped prematurely again.

Doctors deduce a recommendation for therapy. “If patients develop severe headaches in a typical latent period of 5 to 30 days after vaccination, further diagnosis should definitely be made.” explains Matthias Endres, director of the Department of Neurology with Experimental Neurology at Charité. Early and consistent therapy “could completely prevent serious thrombotic events.”
