Researchers have previously noted that the virus that causes covid-19 can persist for many months in HIV-positive patients and for some reason are not receiving treatment. In South Africa, where omicron was first reported, nearly eight million people live with HIV. About a third of them do not take antivirals that can restore the immune system and reduce the amount of virus in the body to such an extent that an HIV-positive person is not infectious.

“Under normal circumstances, if your immune system is fully functional, it will get rid of the virus very quickly.” says Linda-Gayle Bekker, who heads the Desmond Tutu HIV Foundation in Cape Town. “But when someone’s immunity is suppressed, the virus persists. And it’s not like he’s not doing anything, it’s multiplying. And it’s potentially mutating.” she explained.

This case has attracted attention

In the Republic of South Africa, the case of a woman who tested positive for covid-19 for almost eight months attracted attention. The virus in her body has undergone over 30 genetic changes so far.

Tulio de Oliveira, the leader of the team that confirmed the discovery of the omikron variant, states that 10 to 15 similar cases have been observed worldwide. “It’s very rare. But an explanation that people with suppressed immunity can basically stand at the beginning of viral evolution is possible.” admits de Oliveira.

Probable, but not yet proven

Salim Karim, a leading HIV expert and former chairman of the South African government’s covid-19 response committee, also thinks the hypothesis of a relationship between immunocompromised patients and new coronavirus variants is “very likely.”

“But it’s not proven,” Karim points out. “We’ve seen five variations from four different continents. “So it’s outrageous to make a scapegoat from Africa,” adds.

Many HIV-positive people in South Africa are not treated for fear of being accepted into society. “Some don’t want to be tested. They don’t want to know because there are stigmas around HIV.” explains 25-year-old field worker Asiphe Ntshongontshi.

South African experts warn that the vaccination of the population should focus on people with a weakened immune system.
